Physician Assistant Research Survey Link
Dear Potential Participant,
If you are between the ages of 35 and 65, then Linda Cunningham and Erica Yarbrough invite you to participate in an important healthcare study that will assess your level of knowledge about dementia. We are students of the Physician Assistant Program at the University of Findlay who have initiated this research under Christina Gierhart, PA-C and faculty advisor, to identify a need for education in an area of growing concern. With this information we hope to improve educational efforts within the healthcare system that will raise awareness about risk factors and potential complications and provide assistance in planning for future care.
This survey is strictly voluntary and your participation will remain anonymous. Your completion of this survey serves as your consent with the understanding that responses from all participants may collectively be submitted for publication in a peer-reviewed scientific journal. Anonymity is preserved for all participants.

Survey Description
1. This survey consists of 14 questions.
2. This survey should take approximately 10 minutes to complete.
3. This survey has been approved by The University of Findlay Institutional Review Board, which guarantees that research involving human subjects follows federal regulations.
